I cannot recall ever having a fish swallow the sinker - could happen but it would be extremely rare.
I have been fishing for over 70 years in lakes, rivers and the ocean.
I know the old saying "swallowed hook, line and sinker"
The big culprit is fishing line that entangles birds and it does not degenerate.
The fisherman must be responsible for proper line disposal.

In response to the question from RD Spears about eagles eating fishing tackle—they accidentally ingest lead while eating fish that have ingested lead sinkers.
